Schedule Your Appointment
Everything starts here. Schedule your eye exam online or call our office.
If you scheduled online, you'll be prompted to complete a short insurance form after booking. If you scheduled over the phone, you're all set.
If you have a special need, such as if English is not your primary language, you are brand new to contacts, need an evaluation for a child below the age of 7, have a medical emergency, or have complicated visual concerns, please call us to schedule the appointment instead of booking your visit online. This will ensure that we schedule you with enough time to fully address all of your needs. We also have same-day appointments for urgent medical conditions that are not available online.
Complete Your Medical History
Once your appointment is scheduled, you'll be able to complete or update your medical history online.
New Patients
- Leave the email/search field blank.
- Select Search Records for routine eye exams.
Returning Patients
- Enter the email address we have on file.
- We'll automatically load your existing information so you only need to update what's changed.
Download Your Forms
Some forms still need your signature before your visit. Please download the appropriate form below and either bring it to your appointment or fax it to us at 979-985-5305 ahead of time. You will need AdobeReader® to download the forms.
Medical Records & Permissions
If you wish to grant others access to your medical records or if you’d like us to request medical records from your previous eye doctor, please download, complete, and bring the appropriate form with you to your appointment. You can also fax them to us ahead of time at 979-985-5305. You will need AdobeReader® to download and complete the forms.
Important information
- Entering your email address and searching for records will load the information we already have on file for you, so all you need to do is make changes. If you’re an established patient but you either don’t enter your email address or we don’t have one on file, we can merge the information you provide with the records we have, but you’ll still need to enter some information from scratch.
- If entering your email address does not pull your contact information, text us at 979-985-5305 and ask us to re-upload your chart. We automatically upload your chart 24 hours before your appointment.
